          Submitting Payment Requests

          The Employee Action Form is used by Human Resources to process payment requests, reinstate and terminate jobs and terminate employees.

          If you are hiring an employee who is a student, the Fordham Student Employment Policy and Disclosure Form must be included with the Employee Action Form. It ensures that the department and student are aware of Fordham University policy regarding work hours for students.

          All payment requests that fall under a budget associated with the Office of the Provost (any Academic budget) as well as any payments for Faculty members must be sent to the Office of the Provost, Cunniffe House - Second Floor, first. Once approved, an Administrator within the Office of the Provost will forward the request to Human Resources.

          Please view the deadline schedule prior to informing your employees of the expected payment date.

          Additional Payment Requests for Existing Employees 

          The Employee Action Form is the form Human Resources uses to process payment requests for existing employees.

          If someone is currently working for Fordham or has worked for Fordham within the past year, they are considered an active employee and do not need to complete new hire paperwork again.

          Please note that work-study students are not considered University employees in these cases.

          All back-up documentation should be attached.

          Please have the employee supply their Fordham ID Number as the unique identifier instead of the social security number.

          Reinstating an Employee Whose Job Has Ended

          If you are re-hiring/reinstating a non-benefited employee (for example, extending the job of an hourly employee) and they were active within the past year, the Employee Action Form can be used by Human Resources to reactivate an old job using the new hire date.

          All back-up documentation should be attached.

          Please have the employee supply their Fordham ID Number as the unique identifier instead of the social security number.

          Terminating a Non-benefited Employee

          If your non-benefited employee will no longer be working for you, the Employee Action Form has a Leaves/Exit section.

          500 Internal Server Error

          The server encountered an internal error and was unable to complete your request. Either the server is overloaded or there is an error in the application.
